Do You Actually Need Supplements After 40? Here's What the Evidence Says

I've spent the last several weeks tracking every gram of protein I eat, and most days I still come up short. My target is 171 grams. My average is closer to 141. I'm a certified trainer running a business built on evidence-based coaching, and I still can't hit my own number without a shake, a scale, and a spreadsheet. If it's this hard for me, it's probably hard for you too — which is exactly why the supplement aisle feels so tempting.
Walk into any store or scroll any feed and you'll find a hundred products promising to close that gap for you. Most of them won't move the needle at all. A small handful actually will. So I did what I do with every claim I put in front of a client: I pulled the current published research and built a hierarchy, from strongest evidence to weakest. This isn't a sales pitch for any brand, and it isn't a personalized prescription for your body — it's what the literature actually supports for adults over 40 who are training seriously and trying to hold onto muscle.
Protein is still the highest-leverage variable in this whole conversation, and most of us underestimate how much we actually need. The research converges on 1.2–1.6 grams per kilogram of bodyweight per day for adults doing resistance training — well above the 0.8 g/kg RDA that was written for sedentary adults, not people trying to build or preserve muscle.
But the daily total is only half the story, and it's the half that gets all the attention. The detail that actually changes outcomes is the per-meal dose. Because of age-related “anabolic resistance” — a blunted muscle-building response to protein that shows up as early as your forties — you need roughly 30 grams of high-quality protein per meal to fully switch on muscle protein synthesis. That's well above the roughly 20 grams that does the same job for someone in their twenties. One dose-response study in older men found the response kept climbing with intakes up to 40–45 grams per meal, meaning the ceiling is higher than most people assume.
Distribution matters as much as the total. A trial in older men found that adding 25 grams of whey or pea protein at breakfast and lunch — on top of what they were already eating — meaningfully increased muscle protein synthesis over the following hours. Most of us do the opposite: a carb-heavy breakfast, a light lunch, and a protein-loaded dinner, leaving two of three meals under the threshold where protein does its job. Source quality matters too: leucine-rich, animal-based proteins (whey, dairy, eggs, meat) trigger the response most efficiently, and high-quality plant sources like pea protein hold up well. If you add exactly one supplement to your routine, make it creatine monohydrate. It is, without much competition, the best-supported ergogenic supplement in the entire research literature, and the evidence in older adults specifically is remarkably consistent.
One meta-analysis of 357 older adults found that creatine paired with resistance training significantly increased fat-free mass, chest and leg press one-rep max, and 30-second chair-stand performance versus training alone. A more recent review of 33 randomized trials and over 1,000 participants confirmed the same pattern — meaningful gains in lean tissue mass, upper-body strength, and functional sit-to-stand performance — with the benefit holding up regardless of age.
The dosing question has a simple answer: 3–5 grams a day, taken consistently, with no loading phase required. A loading protocol will saturate your muscles with creatine faster, but a steady low dose gets you to the same saturation point within about a month and produces equivalent long-term results. There's even a modest timing edge — a 32-week trial in older adults found that creatine taken immediately after training produced larger lean-tissue gains than the same dose taken before training, though both beat placebo by a wide margin.
On safety: creatine monohydrate has over 500 peer-reviewed publications behind it, decades of use in athletic and clinical populations, and no credible evidence of kidney damage, dehydration, or cramping at standard doses in healthy people. A 2026 trial even found creatine combined with high-load resistance training reduced inflammatory markers and improved cognitive and functional outcomes in older adults, beyond what exercise alone produced. If you're already taking a scoop in your shake, the data says keep doing exactly that.
Leucine
Leucine is the amino acid that triggers the mTOR pathway responsible for muscle protein synthesis, and it has the best evidence of any single amino acid for overcoming anabolic resistance. The effective dose is 3–5 grams per meal, layered onto adequate total protein — it's an amplifier, not a replacement. Older adults likely need more total leucine per day than current guidelines assume, which is one more reason to hit protein targets from leucine-rich sources rather than chase a standalone supplement.
Omega-3 Fatty Acids
Omega-3s show a real but modest effect — small, sometimes inconsistent strength benefits requiring 2+ grams per day sustained for six months or longer. Evidence is stronger in women than men, and most reliable when combined with resistance training. A reasonable add-on for general health, not a primary lever for muscle.
HMB and Vitamin D
HMB (a leucine metabolite) shows small, consistent effects on muscle mass and strength, but earns its keep mainly in clinical and frail populations — sarcopenic or bed-bound older adults, or anyone for whom resistance training isn't currently feasible. For a healthy, already-training adult, its benefit over food and creatine alone is likely marginal. Vitamin D helps only if you're actually deficient; in people with normal levels, supplementing it produces no measurable improvement in muscle mass, strength, or function. Correct a real deficiency for your general health — don't expect it to build muscle.

Here's where I have to stay in my lane. I'm a certified personal trainer, not a physician or a registered dietitian, and I'm not going to pretend a newsletter can tell you what's safe for your specific bloodwork. This matters more than it sounds like it should.
An estimated 12–14% of adults over 40 have undiagnosed kidney disease, and that number is higher if you have hypertension or diabetes. A 15-year study following more than 8,000 adults with non-dialysis chronic kidney disease found the risk of dialysis initiation rose progressively as protein intake climbed above roughly 1.0 gram per kilogram per day, with the sharpest increase above 1.6 g/kg. Current kidney-disease guidelines specifically advise against exceeding 1.3 g/kg/day at CKD stage 3 or higher. If you don't know your kidney function, pushing your protein intake up without that information isn't a risk worth taking blind.
For healthy adults without kidney disease, the picture is reassuring: multiple systematic reviews have found high-protein diets up to 1.5–1.6 g/kg/day do not adversely affect kidney function. There's also a cardiovascular signal worth knowing — one large cohort study linked very high intake, above roughly 1.8 g/kg/day, to increased cardiovascular risk specifically in adults over 55, though not in younger adults. None of this means protein is dangerous. It means the 1.2–1.6 g/kg range recommended here is the range with evidence behind it, and going meaningfully beyond it is where you want a doctor's input, not a trainer's guess.
One more note: protein supplements can cause GI discomfort in a subset of people, especially at higher single doses. If that's you, start smaller and spread it out — it's a tolerability issue, not a safety one, but worth knowing before you double your shake size overnight.
Strip away the marketing and the evidence supports a short list: protein, dosed at 1.2–1.6 g/kg/day and spread across meals at roughly 30 grams each, and creatine monohydrate at 3–5 grams a day, taken consistently. Everything else — leucine, omega-3s, HMB, vitamin D — is a smaller, optional add-on that works best layered on top of those two, not instead of them. And none of it works without the resistance training you're already doing, which remains the actual stimulus that protein and creatine are amplifying.
I'm not chasing a supplement stack, and I don't think you need one either. I'm making sure the two things with real evidence behind them are actually dialed in before I spend another dollar on anything else — and I'd rather you check with your doctor about your own kidney numbers than take a dosing range from a newsletter and run with it blind.
